A Program Developed For Patients With Catastrophic Injuries
Individuals suffering a catastrophic injury or illness need specialized ongoing care to help them return to the best lives possible. Santa Clara Valley Medical Center, a leader in rehabilitative care for over 40 years, has partnered with Learning Services, Care Meridian and Rehab Without Walls, to provide a program designed to improve help for patients and their families through the first year after injury.
Needs will change over the course of the year, NEURO-NET participants will work with an experienced nurse case manager who will provide real time help with coordinating care, re-entry into the community, advocate for their treatment needs, and education when it matters most.
This service will be at no cost to the patient, or their insurance provider. This pilot program is funded by a generous innovation grant through the VMC Foundation.
The goal of the program is to create a new system of integrated care, to better help patients and families navigate through their recovery journey.
We will be gathering outcome information to help us better understand the things that are most helpful in keeping patients healthy, happy and engaged with the community. At the same time, participants in the study receive help along every step of the journey.

Expert help navigating recovery for one year from an experienced nurse case manager including:
You will have regular contact with a nurse case manager by phone.
If you have questions or concerns that are not medical emergencies, you can call your nurse case manager for advice anytime.
Your case manager will help coordinate all your transitions of care from one facility to another, or to home.
Your case manager is available to visit you in person if needed. The case manager will be available to attend important medical appointments with you.
Your case manager is an expert in many community resources and will help facilitate accessing these resources.
Your NEURO-NET case manager is intended to be a trusted partner in rehabilitation with you and your family, your health insurance and the facilities and providers where you receive medical treatment.
