A One Year Pilot Program Developed For Patients With Catastrophic Injuries
WHAT IS NEURO-NET?
Individuals suffering a catastrophic injury or illness need specialized ongoing care to help them return to the best lives possible. Santa Clara Valley Medical Center, a leader in rehabilitative care for over 40 years, has partnered with Learning Services, Care Meridian and Rehab Without Walls, to provide a program designed to improve help for patients and their families through the first year after injury.
Needs will change over the course of the year, NEURO-NET participants will work with a experienced nurse case manager who will provide real time help with coordinating care, re-entry into the community, advocate for their treatment needs, and education when it matters most.
This service will be at no cost to the patient, or their insurance provider. This one year pilot program is funded by a generous innovation grant through the VMC Foundation.
OUR GOAL FOR YOU
The goal of the program is to create a new system of integrated care, to better help patients and families navigate through their recovery journey.
We will be gathering outcome information to help us better understand the things that are most helpful in keeping patients healthy, happy and engaged with the community. At the same time, participants in the study receive help along every step of the journey.
The journey starts with NEURO-NET.
